Grants to Wisconsin nonprofit organizations in eligible areas for projects that improve the health and well-being of local residents. Funding is intended to support projects that address the following areas: oral health, mental health, nutrition, physical activity, social determinants of health, chronic disease prevention and management, alcohol and other drug use, and access to health services. Requests for capital projects will be considered. Eligible projects benefit those who live and work in Washington County.
The Healthy Community Fund’s highest funding priority is given to programs or projects that address social determinants of health and/or target underserved populations. Funding is available for organizations in good standing that represent a range of demographics within Washington County.
Some examples of program areas that will be considered for funding include, but are not limited to, community collaborations or initiatives; child, youth and families; elderly; transportation; access to care; and/or environmental change.

The minimum dollar amount for a request is $10,000.
Multi-year projects are not guaranteed funding for consecutive years.
